When creating a form, it's important to use the appropriate field types to gather the necessary information efficiently. Here's a guide on how to use different field types effectively:


  • Header

Use this to clearly indicate the name or title of your form.

  • Paragraph

For writing a short or long explanation regarding your form

This form is designed to assess your physical activity readiness. Please answer each question honestly.

  • Text Field

Use this for questionnaires where a short answer is expected.

  • Text Area

Use this for questionnaires where a long answer is expected

For example

Please describe any medical conditions you currently have and any medications you are taking.

  • Number

Use this for questionnaires where the answer must only be a number.
